Bridging the Gap Between Hospital and Home

For an individual experiencing homelessness, being discharged from a hospital is often just the beginning of another crisis. Where do you go to recover when you don't have a home? Returning to the streets or a shelter can make recovery nearly impossible, leading to complications, infections, and costly, preventable readmissions to the hospital.
Infinity NP’s Recuperative Care Program breaks this cycle. We provide a safe, clean, and supportive environment for individuals to rest, recover, and heal after a hospital stay, creating a vital bridge to better health and permanent housing.

What is Recuperative Care?

Our Recuperative Care program (also known as medical respite care) is a short-term residential service that allows individuals experiencing homelessness the opportunity to recuperate in a safe environment. We provide the stability and support needed to ensure a person can follow their medical discharge plans, preventing further health complications.

Our Program Provides:
  • A Safe & Clean Bed: A stable environment to rest and focus on healing, away from the stress of the streets.
  • Nutritious Meals: Three healthy meals a day to support physical recovery.
  • Transportation: We ensure clients can get to and from essential follow-up medical appointments.
  • Care Coordination: We work closely with clients and their case managers to connect them with primary care, behavioral health services, benefits enrollment, and housing resources.
  • A Supportive Community: An environment of dignity and respect where individuals can rebuild their strength and plan their next steps.

Level of Care

Important: Our Level of Care is also known as Social Respite

To ensure the safety and success of our clients, it is important to understand the specific level of care we provide. Infinity NP operates a social respite model, which means we are not a licensed medical facility.
  • We do not provide direct clinical or nursing care. We do not have doctors or nurses on-site.
  • Our staff provides encouragement, support, and transportation, but cannot administer medications or perform medical tasks like changing wound dressings.
  • This program is not a substitute for a hospital, skilled nursing facility (SNF), or long-term care.

Who is Eligible for our Program?

Our Recuperative Care program is designed for individuals who are:
  1. Experiencing homelessness or housing instability.
  2. Being discharged from a hospital or emergency department and need a safe place to recover.
  3. Medically stable and able to manage their own basic needs.
  4. Able to manage their own Activities of Daily Living (ADLs), such as bathing, dressing, and using the restroom independently.
  5. Able to self-manage all of their own medications.
A referral from a hospital, clinic, or healthcare partner is required for admission.

Our ultimate goal is not just to help individuals recover from an acute illness or injury. It's to stop the revolving door between the streets and the hospital. By providing a stable foundation for health, we empower our clients to connect with the resources they need to secure permanent housing and achieve long-term stability.

For Individuals Needing Care:
If you are currently hospitalized and believe you qualify for our program, please speak with your hospital's case manager or social worker and ask them to make a referral to Infinity NP’s Recuperative Care Program.
